Our knowledgeable team of specially trained hospice professionals works to continually maintain a personalized program of care that meets your unique needs. We coordinate our services to complement the care already being provided by family, friends, and the medical professionals you know and trust.
Medical Director
Oversees the plan of care and works collaboratively with your personal physician.
Case Manager
A registered nurse that is specially trained to provide comfort care and pain management. Coordinates responses to patient needs with all members of the hospice team, physicians, nursing home and assisted living staff, and caregivers.
Home Health Aide
Assists with daily living activities, such as personal grooming, bathing, or just taking the time to sit and talk.
Social Worker
Provides emotional support as well as assistance in dealing with end of life concerns and accessing community resources that can provide additional help.
Spiritual Care Coordinator
Ensures that spiritual needs are addressed and, when needed, acts as a liaison between you and your faith community.
Volunteers
Specially trained, caring individuals dedicated to providing companionship and support.
