Patients and Families
Encircling Arms Around a Family In Need
In these moments it is not unusual to find yourself and family members
struggling in a sea of emotions. In fact, according to experts in Hospice
and Palliative Care, it is not unusual to experience, among other things,
bouts of anger, depression, fear and uncertainty about what the days
ahead hold. We understand the emotional stress and daily hurdles
that patients and their families experience when facing these challenges.
We are committed to giving you and your family or loved ones the physical,
emotional and spiritual support you need and deserve
during this time.
At LightBridge Hospice, providing for your comfort, dignity and peace of
mind is not only our mission but our passion.
Every patient at LightBridge is served by an interdisciplinary team that
includes a Medical Director, RN Case Manager, Social Worker, Spiritual
Care Counselor, Certified Home Health Aide and Volunteer. Other disciplines
are included in the team as needs arise. Of course, we
always honor patient
choice when making team assignments. Our team members work
collaboratively with one another and a patient’s primary care physician
and caregivers to provide pain control and symptom management while
offering comfort, honoring patient preferences and respecting patient dignity.
